Job Description:
A company is looking for a Care Coordination Lead to manage
their care coordination function and ensure efficient processing of
client requests.
Key Responsibilities
Lead the care coordination function, developing workflows and
processes from scratch
Manage the end-to-end process for care coordination requests,
ensuring timely and high-quality processing
Collaborate with cross-functional teams to create systems that
streamline care coordination
Required Qualifications
5+ years of experience in care coordination, case management, or
operational leadership within the behavioral health field
Proven track record of building processes or programs from scratch,
ideally in a start-up setting
Strong understanding of HIPAA and healthcare regulations to ensure
compliance
Experience in tracking KPIs and using data for continuous
improvement
California residency and CCM or CPHQ credential preferred, but not
required
